• Your preferred contact information (Player AND Guardian)
• Doctor contact information
• US Lacrosse Number valid through June 30,2023 (for Spring Season) - Registration will take you to the US Lacrosse website to update your membership automatically if needed, or create a new one. This is used for insurance purposes. New ones are $35, and are a separate cost from our registrations.

There is a $100 discount when registering 3 siblings and a $200 discount for 4 siblings.
